This script requires a blank white image with an active mask and no floating objects or masks in the alpha channel (save mask as..).  The script uses the supplied silhouette mask to generate stages of the cut_out effect and is finished when a message box appears with the following information:

Import or fill in a background.  Load the "shape" mask to prep the recessed area with the desired effect (lighten, darken, color, etc.), remove mask.  Ctrl-V to paste a shadow map, multiply merge it and combine layers to background.  Lastly, load "edge" mask and paint on a light color for a catchlight effect.

The above image was fashioned using zone masks (level threshold views) from the isomap script and the above cut_out script to make a multi-level cut-out effect.
